Registration Forms

We welcome you to our Faith Formation programs! Registation for one child is in the first part of the form. Additions children can be listed at the end of the form.
*Registration fee of $10 per student or $25 per family will be required. Payment can be made on-line by clikcing the donate button and choosing "Faith Formation", be brought to first day of class or can be mailed into: Sacred Heart St. Franices de Sales, Faith Formation Registration, 238 Main Street, Bennington, VT 05201. Please contact 802-442-3141 with any questions.
**First Eucharist and Confirmation students who were not Baptized at Sacred Heart St. Francis of St. John the Baptist church will need to provide a copy of the child's baptism certificate.
